From March 11 to 19, a delegation of 14 members from the
MOH Office for Healthcare Transformation (MOHT) in Singapore traveled to France to explore innovative practices in elderly care and frailty detection. This study mission provided an opportunity to examine various geriatric care models and initiatives through site visits and discussions with experts.
Day 1: Immersion at La Grave Hospital (CHU de Toulouse)The mission began with a presentation of the
ICOPE program and its associated digital tools, such as ICOPE Monitor and the telemonitoring platform. The afternoon was dedicated to visiting the outpatient department and the Clinical Research Center, offering insights into innovations in frailty screening and management.
Day 2: Exploring Regional Initiatives in ToulouseThe delegation visited the South Toulouse Territorial Professional Health Community (CPTS), a coordinated care model focused on prevention and improving healthcare access.
A visit to Hôpital Garonne provided a deeper understanding of long-term care and geriatric rehabilitation approaches, particularly for patients with Alzheimer’s disease.
Day 3: Visit to Clinique des MinimesThe delegation discovered a unique multidisciplinary model of aging care in France, bringing together all geriatric specialties on a single site to prevent loss of autonomy.
Day 4: Immersion in Île-de-FranceThe final day started with a meeting with the
Gérond’if teams on frailty detection in Île-de-France. Experts, including
Professor Olivier Hanon and
Dr. Philippe Charru, presented the scientific and organizational challenges of geriatric care.
The delegation then visited CPTS Paris 14, where
Dr. Marie-Laure Alby explained the implementation of the ICOPE program. The mission concluded with an immersion in the emergency unit at Hôpital Cochin, where
Dr. Matthieu Piccoli presented the PAGUC project, aimed at improving elderly patient care in emergencies.
ConclusionThis study mission provided the MOHT delegation with a comprehensive view of best practices in France regarding elderly care and frailty detection. The discussions with healthcare professionals and the discovery of innovative models will likely inspire new initiatives in Singapore.
